We evaluate and treat a wide range of neurological conditions, including migraines and headaches,seizures and epilepsy, memory loss and dementia, multiple sclerosis (MS), neuropathy, tremors, dizziness,and other disorders of the nervous system.

Some insurance plans require a referral from your primary care provider, while others do not. We recommend checking with your insurance company prior to scheduling your appointment.

Please bring a photo ID, insurance card, referral (if required), a list of current medications, relevant medical records or imaging, and any questions of concerns you would like to discuss with the provider.

Please call our office for medication refills and allow at least 72 hours for them to be processed. For safety and continuity of care, patients are generally required to follow up with their provider at least every six (6) months to remain eligible for refills. Certain medications may have additional requirements.
